- Teach the skills you need to manage your own nutrition care
- Helps to lower chances of health problems and illness by adopting healthy lifestyle changes
- A nutritional assessment will identify a nutrition care plan based on what you are currently eating and what changes are needed
- Follow-up appointments will help to measure your progress and review what you’ve learned. Adjustments will be made to your meal plan and any additional information shared as needed.
